Centers Health Care is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide direct, high-quality nursing care to patients under the supervision of a Registered Nurse (RN) and/or physician, ensuring their physical, emotional, and psychosocial needs are met. You will administer medications and treatments in accordance with physician orders and established protocols, monitor and document vital signs, perform wound care, assist with diagnostic tests, and respond promptly to changes in patient condition. You will collaborate closely with an interdisciplinary care team—including RNs, physicians, therapists, and social workers—to support individualized care plans that promote optimal outcomes, safety, and comfort for each patient. Documentation of all care provided will be completed accurately and timely using electronic health record (EHR) systems, in compliance with regulatory and organizational standards.

As part of Centers Health Care, you will work in a fast-paced clinical environment where strong communication skills, clinical judgment, and attention to detail are essential. You will provide patient and family education on diagnoses, medications, treatments, and self-care, while demonstrating empathy, respect, and cultural sensitivity. You will help maintain a clean, safe, and infection-controlled environment, adhere to all infection prevention policies and procedures, and follow best practices in quality and patient safety. This position requires reliable attendance, the ability to prioritize multiple tasks, and a commitment to ongoing professional development.
