Mantey Heights Rehabilitation & Care Center, part of Stellar Living in Grand Junction, CO, is seeking a compassionate Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. In this role, you will provide direct, hands-on nursing care to residents in our assisted living, memory care, and rehabilitation environment, supporting their comfort, safety, and overall wellness. You will administer medications and treatments as ordered, monitor vital signs and changes in condition, and collaborate closely with RNs, providers, caregivers, and therapy staff to implement individualized care plans. The LPN will document all care accurately in the electronic medical record, follow regulatory and facility policies, and maintain a high standard of infection control and resident safety. You will provide respectful assistance with activities of daily living as needed, support residents with mobility and rehabilitation goals, and respond promptly to call lights and urgent needs.
