In this role, you will provide high-quality, patient-centered care in a collaborative, innovative environment that supports both clinical excellence and professional growth. As a key clinical provider within our Healthcare & Medical Services department, you will perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans that reflect evidence-based best practices and AlignMed’s commitment to outstanding patient outcomes.

Working closely with physicians, nurses, and a broad interdisciplinary care team, you will conduct detailed patient histories and physical examinations, order and interpret diagnostic tests, prescribe medications within your scope of practice, and adjust treatment plans based on patient response and clinical guidelines. You will play an active role in patient education, helping individuals and families understand diagnoses, medications, lifestyle modifications, and preventive care strategies that promote long-term wellness. Your clinical judgment, clear communication, and ability to build trust with patients will be central to your success in this position.

The Nurse Practitioner will also participate in care coordination across different settings to ensure continuity and a seamless patient experience. This includes collaborating with referring providers, specialists, and practice management teams to close care gaps, optimize follow-up, and support population health initiatives. You will document all encounters thoroughly and accurately in the Electronic Health Record (EHR), ensuring compliance with clinical, regulatory, and quality standards, including HIPAA and applicable state and federal regulations.
