Position Highlights:

* Position: Nurse Practitioner (NP) - Post Acute Care

* Location: Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital).

* Full Time/Part Time: Full-Time, 40 hours per week

* Hours: Monday - Friday between 8:00 a.m. and 5:00 p.m.

* Potential for weekend hours in the future

* No evenings or holidays

What you will need:

* License: Nurse Practitioner (APN) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

* Education: Completion of Master's level program in specialty area

* Certification: BLS/CPR certification required

* Experience: Minimum of three years Nurse Practitioner experience. Experience with advanced directive and goals of care.

* Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease

* Promote wellness and prevent illness and injury in various settings for individuals, families, and communities

* Work autonomously and in interdisciplinary teams as resources and consultants

* Positively contributing to safety outcomes and promoting high quality patient experience
