CenterWell Home Health is seeking a compassionate and skilled Occupational Therapist for in-home patient care within our Healthcare & Medical Services department. As an Occupational Therapist, Home Health, you will deliver individualized, evidence-based therapeutic interventions in patients’ homes to restore function, enhance safety, and promote independence in daily activities. You will complete comprehensive evaluations, develop and implement goal-oriented care plans, and provide hands-on therapy focused on functional mobility, self-care, home safety, and adaptive techniques. Collaborating closely with an interdisciplinary team—including nurses, physical therapists, speech therapists, social workers, and physicians—you will coordinate care, monitor progress, and make timely adjustments to treatment plans to drive optimal outcomes. You will educate patients and caregivers on home exercise programs, energy conservation, safe transfers, use of adaptive equipment, and fall prevention strategies, while documenting all care accurately in the electronic medical record and maintaining regulatory and agency compliance.
