Job Description

Evaluates, plans, treats and implements care for patients in accordance with professional standards of the American Physical Therapy Association and policies and procedures of Essentia Health.

Education Qualifications:

Bachelor's Degree,

Master's Degree , or Doctorate Degrees from a physical therapy accredited program

Key Responsibilities

Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload. Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties Demonstrates

appropriate communication ,

professionalism

and supervision of support staff (licensed assistants, aides, volunteers) Collaborate with a multidisciplinary care team to ensure

optimal

clinical and financial outcomes. Work closely with Essentia Health

leadership

and coordinate with referring physicians to ensure cohesive patient care. Compl y

This full-time position is scheduled for 40 hours per week, working four to five days per week. One to two days will include evening hours ending at 6:00–6:15 p.m., with remaining days concluding between 5:00–5:30 p.m. Scheduling options include four 10-hour shifts or five later weekday shifts. Licensure/Certification Qualifications:
