Job Summary

The physical therapist evaluates, plans, and implements rehab programs to enhance mobility, alleviate pain, boost strength, and prevent or correct deformities. Services are delivered cost-effectively, with a focus on quality and customer satisfaction, while adhering to all regulatory standards.

Core Responsibilities and Essential Functions: Patient Assessment: Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient’s physical therapy needs.

Create and adjust care plans with input from the patient and/or family.

Develop realistic discharge plans incorporating patient and family goals.

Treatment Planning and Provision of Care: Implement and adjust physical therapy treatment plans.

Select appropriate activities to help patients achieve their goals.

Revise goals and plans with input from patients and families.

Provide ongoing education and training to patients and families.

Give discharge instructions and referrals to community resources as needed.

Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.

Documentation: Document evaluations, treatment plans, and each session according to departmental guidelines.

Complete and submit all required documentation, including family education and discharge information.

General Requirements: Uphold ethical standards and demonstrate integrity and honesty.

Embrace diversity and collaborate effectively.

Maintain confidentiality of patient information.

Adhere to attendance, punctuality, and dress code policies, including wearing an ID badge.

Communicate clearly and effectively with all team members.

Attend staff meetings, in-services, and other required activities.

Organize workload to meet responsibilities timely and safely.

Use materials and equipment safely and cost-effectively.

Follow infection prevention protocols.

Customer Service: Lead with vision, technology, innovation, and excellent customer service.

Required Minimum Education:

Graduate from an accredited physical therapy program Required and

Bachelor's Degree in Physical Therapy Required or

Master's Degree in Physical Therapy Required or

Doctorate in Physical Therapy Required

Required Minimum License(s) and Certification(s):

Physical Therapist 1.00 Required

Basic Life Support 2.00 Required

Required Minimum Skills:

Effective communication skills, customer service focused, team oriented, and basic computer skills.
