Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • No new graduates. Minimum recent clinical experience as required by client. (Client requires experienced clinicians.)
  • Experience in outpatient physical therapy setting preferred.
  • Demonstrated ability to evaluate patients, develop and implement treatment plans, perform functional outcome documentation, and write letters of medical necessity.
  • Ability to supervise and delegate to discipline-specific therapy staff and to participate in student education as assigned.
  • Maintain professional licensure and complete state-mandated continuing education per State Practice Act.
