Previous home health experience required, including use of HCHB electronic medical record. BLS certification from AHA or ARC may be required. Experience providing patient assessments, developing home-based treatment plans, and documenting progress in home health settings.

Preferred Qualifications Prior experience working for LHC or similar home health organizations. (Preferred) Experience managing a caseload across regional coverage areas such as Elizabethtown-Hardin County and Hardinsburg-Meade County. (Preferred)

Assignment Details Shift hours: 08:00 to 16:00 as documented by client schedule. Workdays: Monday through Friday. Weekly guaranteed hours: 32. Productivity expectation: 30 points. Coverage area includes Elizabethtown-Hardin County and Hardinsburg-Meade County. Mileage reimbursement provided between patient visits only, from first patient to last patient of the day. On-call and weekend requirement: no.
