Minimum Requirements Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Oregon Physical Therapist license required to start the assignment. Current BLS (Basic Life Support) certification from AHA or ARC preferred. If required by client, BLS must be valid prior to start. Experience providing home health physical therapy services. Client prefers clinicians with prior home health caseload management, documentation experience, and ability to work independently in community settings. Valid driver’s license and reliable transportation for travel within Jackson County. Client requires clinicians to travel to patient homes across the coverage area. Ability to perform patient assessments, develop skilled treatment plans, and provide skilled interventions for adult and geriatric populations. Prior experience with mobility training, fall prevention, and ADL retraining preferred. Demonstrated proficiency with electronic medical records documentation. Client may require timely completion of visit notes and communication with interdisciplinary team.
