Trilogy Health Services is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team, dedicated to enhancing the lives of seniors in our long-term care and senior living communities. In this role, you will provide hands-on, evidence-based physical therapy services to residents with diverse medical and functional needs, with a strong focus on maximizing independence, mobility, and quality of life.

As a Physical Therapist with Trilogy, you will be responsible for completing comprehensive evaluations, identifying functional limitations, and developing individualized treatment plans tailored to each resident’s goals, abilities, and personal preferences. You will deliver skilled interventions including therapeutic exercise, gait and balance training, transfer training, neuromuscular re-education, and pain management techniques within your scope of practice. You will also recommend and train residents in the safe use of assistive devices and adaptive equipment.

You will document evaluations, daily treatment sessions, progress notes, and discharge plans accurately and promptly using our electronic medical record system, ensuring compliance with clinical, regulatory, and payer requirements. You will collaborate closely with nursing, occupational therapy, speech therapy, and other members of the interdisciplinary care team to coordinate holistic, resident-centered care, and you will actively participate in care conferences and goal-setting discussions.

Education and support are central to this position. You will provide clear instruction to residents, families, and caregivers on home exercise programs, fall-prevention strategies, proper body mechanics, and techniques to maintain gains achieved in therapy. You will serve as a resource on mobility, safety, and functional independence for both residents and staff.
