Good Samaritan Health Center is seeking a compassionate, detail-oriented RN, Minimum Data Set (MDS) nurse to support our mission of providing comprehensive, holistic healthcare services to underserved communities. In this role, you will be responsible for coordinating and completing accurate, timely Minimum Data Set (MDS) assessments for residents, ensuring regulatory compliance while promoting meaningful health and wellness outcomes.
As the RN MDS nurse, you will lead and manage the Resident Assessment Instrument (RAI) process, gathering clinical information from the interdisciplinary team, reviewing medical records, and collaborating closely with physicians, therapy, social work, and other nursing staff. You will translate clinical findings into precise MDS documentation, supporting individualized care plans that reflect each resident’s physical, emotional, social, and spiritual needs. Your work will directly influence quality measures, reimbursement accuracy, and the overall resident experience.
Working within the Healthcare & Medical Services department at Good Samaritan, you will serve as a clinical resource and advocate, educating staff on MDS requirements, changes in regulations, and best practices in documentation. You will participate in care conferences, contribute to the development and review of care plans, and help monitor resident outcomes to ensure that care is both person-centered and aligned with Good Samaritan’s values of compassion, integrity, and community engagement.
This position requires strong assessment skills, critical thinking, and the ability to interpret and apply CMS regulations and state survey standards. You will routinely review charts, validate clinical documentation, and analyze data related to quality indicators and case mix. You will work closely with leadership to support audit readiness and continuous quality improvement initiatives, identifying opportunities to enhance resident care while maintaining compliance and optimizing the use of resources.
Good Samaritan offers a collaborative, mission-driven environment where your professional expertise as an RN MDS nurse will be valued and supported. You will have opportunities for ongoing education and professional growth, including development in advanced MDS practices, quality improvement, and community health initiatives. While the role is primarily on-site to support residents and staff, there may be limited flexibility for remote work related to documentation review and data entry, depending on operational needs.
Candidates should be comfortable working with electronic health records (EHR), managing multiple deadlines, and communicating effectively with diverse teams and families. A strong commitment to serving vulnerable and underserved populations is essential. By joining Good Samaritan, you will help ensure that every resident’s story is fully captured in the MDS process and that care plans are tailored to promote dignity, independence, and overall wellness.
We offer competitive compensation, comprehensive medical, dental, and vision coverage, life insurance, retirement benefits, and opportunities to participate in community outreach and wellness programs. This is a full-time RN role for a dedicated professional who wants to use their MDS expertise to make a tangible difference in patient and community health.
