BLS certification (Must be AHA) Current nursing license with Nursys verification within 30 days prior to start License/Certificate verification within 30 days prior to start Completion of Ascension Core Competencies (HIPAA, Joint Commission, NPSG, OSHA Topics) with minimum 80% passing score Ascension EKG Exam with passing score of at least 80% Ascension Onboarding Requirements Education Attestation Ascension Nursing Contract Worker Onboarding Ascension Nova StatStrip pre-hire modules MOAB Moderate Risk De-Escalation Training Ascension Cerner Test-Out with signed attestation Documentation of two MMR vaccines or positive IGG Titer Fit Test (preferred prior to start, can be completed onsite) Hepatitis B vaccination series completion or signed declination Health Statement or completed physical exam prior to first assignment, updated annually Driver's License/State ID (must not be expired and address must match) 2 Step PPD OR IGRA (Quantiferon/Tspot) with required timing and annual TB Questionnaire Documentation of two Varicella vaccines or serological evidence of immunity Tdap valid within 10 years Ascension Profile (Resume) Two professional references prior to first assignment Ascension Criminal Background Check Results including Social Security Trace, OIG Search, National Sex Offender Ascension EPLS run within 90 days of start Ascension Skills Checklist completed within one year of start and updated annually Ascension 12 Panel Drug Screen within 30 days prior to start and updated annually Ascension Education Verification within 90 days of start Ascension Confirmation of Assignment upon initial assignment and updates Ascension Employment Verification for past seven years, updated annually Schedule Information
