Monday, Wednesday, Friday: 8 AM - 5 PM; Tuesday, Thursday: 9 AM - 6 PM

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patients’ needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services to assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The therapist acts as an educator for patients, families, students, and coworkers while performing other professional duties associated with patient care and therapy service development.

Responsibilities
  • Complete comprehensive evaluations of assigned patients.
  • Utilize clinical reasoning to develop appropriate treatment plans and goals.
  • Ensure timely and accurate medical record documentation, inclusive of all required assessments and charge entries.
  • Participate in student programs as assigned, providing supervision and evaluation support.
  • Create and actively pursue an individualized professional development plan.
  • Maintain professional licensure in good standing and adhere to ethical standards.
  • Contribute to organizational performance improvement initiatives.
  • Promote patient and employee safety in alignment with organizational standards.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an accredited school.
  • 1+ year of outpatient clinical experience.
  • CPR Certification.
  • Active and applicable state licensure as a physical therapist.
  • Previous experience with our agency is highly desired.
