Seeking a dedicated Physical Therapist (PT) to provide rehabilitation services in a Subacute and Skilled Nursing Facility setting. The PT will evaluate residents, develop individualized treatment plans, and provide therapy focused on improving mobility, strength, balance, and functional independence. Must-Have Skills:
✔ Active California Physical Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Ability to work in Skilled Nursing / Subacute settings
✔ Excellent communication and teamwork skills
✔ Accurate clinical documentation skills Experience Required:
✔ Minimum 3 year of PT experience required
✔ Skilled Nursing / Subacute experience preferred
✔ New graduates encouraged to apply based on experience level
Key Responsibilities
• Evaluate patients and create individualized treatment plans
• Provide therapeutic exercise, gait, balance, and mobility training
• Monitor progress and modify treatment plans as needed
• Educate patients and caregivers on exercises and safety strategies
• Maintain timely and accurate documentation
• Collaborate with rehab team and interdisciplinary staff
• Promote safe functional independence and quality outcomes
