Must have ARRT certification Must have BLS certification License/Certificate Verification required within 30 days of start date and updated upon expiration Fit Test (N95) within 365 days to start date and updated annually Respirator Medical Clearance (RMC) Attestation required if fit test is required Documentation of MMR vaccination or immunity TB Test documentation within 3 months of start date and updated if >6 month break Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break MGB Resume including past 7 years of work experience 2 references within 4 years of submission, minimum 1 supervisor or above in skillset MGB Skills Checklist completed within 1 year of submission MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions MGB Education Verification must verify highest level education achieved MGB SSN Trace must be run with Criminal check MGB National Sex Offender Check within 30 days of start and updated annually MGB Exclusion Checks within 30 days of start and annually Schedule Information
