Heritage Park Care Center is seeking a compassionate, detail‑oriented Registered Nurse to join our Healthcare & Medical Services team, providing high‑quality clinical care to residents in a collaborative, patient‑centered environment. As a key member of our interdisciplinary care team, you will conduct comprehensive patient assessments, develop and implement individualized care plans, administer medications and treatments in accordance with physician orders and nursing standards, and closely monitor patient responses and conditions. You will be responsible for accurately documenting all care, observations, and interventions in electronic medical records, ensuring regulatory compliance and continuity of care. The Registered Nurse will coordinate with physicians, therapists, social workers, and nursing assistants to optimize patient outcomes, participate in care conferences, and provide clear, empathetic communication with residents and their families about conditions, treatment options, and care goals. You will supervise and mentor licensed practical nurses and nursing assistants, delegating tasks appropriately, reinforcing evidence‑based practices, and supporting a culture of safety, respect, and accountability. Infection prevention and control will be a core part of your role, including adherence to all facility protocols, timely reporting of changes in condition, and participation in quality improvement initiatives designed to enhance resident safety, comfort, and satisfaction. This role requires strong clinical judgment, the ability to prioritize multiple patients and tasks in a fast‑paced setting, and comfort with using technology for documentation and communication.
