Member of interdisciplinary team providing and managing patient care using nursing process. Promotes evidence-based practice through nursing protocols and procedures. Assesses patient status via interview, observation, and physical assessment. Develops individualized care plans collaboratively with patient/family and healthcare team. Uses appropriate interventions to achieve expected patient outcomes. Evaluates and documents patient progress throughout shift. Modifies care plans based on ongoing evaluation.
