Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program. Active MA Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home Health experience preferred. Candidates with Hospital or SNF experience will be considered. Experience using electronic medical records. Familiarity with Homecare Homebase preferred. Ability to meet productivity target of 6 points/day (30 points/week). Valid driver's license and reliable transportation for patient visits within a manageable territory. May consider a PT/COTA pair; COTA applicants may be considered when submitted with a PT.

Facility type: Home Health. Shift name: Days. Schedule options: 5x8 (Monday–Friday) or 4x10s. Primary coverage area includes Northbridge, Upton, Uxbridge, Mendon, Millville, Blackstone, and Bellingham. Territory is manageable with street parking and driveways available. EMR: Homecare Homebase. On call: None.
