Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Arizona state PT license required to start the assignment. BLS certification from AHA or ARC may be required. Experience in outpatient physical therapy settings preferred. Demonstrated ability to complete comprehensive evaluations, develop and implement individualized treatment plans, and progress patients toward discharge within established lengths of stay. Proficiency with documentation including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in compliance with state and federal guidelines; experience with electronic medical records preferred. Ability to supervise and delegate to discipline-specific therapy staff and to participate in student clinical education when assigned. Maintain professional licensure and meet state-mandated continuing education requirements. Adherence to professional codes of conduct, patient confidentiality, and safety policies. Comfort working in a rural clinic environment.

Assignment Details

Facility type: Outpatient clinic. Shift pattern: 5x8 schedule. Assignment duration: 13 week assignment; client indicates a 6 month duration is desired. Number of positions available: 1.
