Join our team as a PACE Primary Care Physician and help older adults live independently while receiving high-quality, all-inclusive care.What You'll Do- Serve as primary care physician for PACE participants (55+)- Provide preventative, acute, and chronic care with a focus on aging with dignity- Collaborate with the Interdisciplinary team to coordinate holistic care plans. The interdisciplinary team includes on-site social work, physical and occupational therapy, a registered dietician, a day center coordinator, home care, transportation, and more.- Build trusted relationships with participants and their familiesRequirements:- MD/DO with interest in geriatrics- BE/BC in IM, FM, or Geriatrics- Active CA license + DEA- Meets 42 CFR Part 410.20- Minimum 1 year experience with frail/elderly population
